New and Returning Athletes are to complete this form to request new or additional items of uniform.

All Athletes are expected to have a complete uniform kit respective to their sporting requirements.

New Athletes will receive the following (below), returning Athletes should have the complete uniform kit from previous years.

If returning Athletes require replacement items or additional items these will incur addititional costs on top of the reduced returning Athlete Levy (see costings below)
