This form is for the reimbursement of levies paid to the South West Sydney Academy of Sport for camps which have been cancelled.

Funds will be reimbursed to the payee on the next fortnightly pay cycle.

I have double checked the bank details provided are correct, and the above details are for the nominated bank account I wish for the funds to be reimbursed into.

I declare that I have paid in full the original camp levy and as such am requesting the full amount to be reimbursed. 

I understand that if there are any other levies (athlete, uniform or camp) outstanding to SWSAS, that the reimbursement may first be applied to that outstanding amount, and any remaining reimbursement will be forwarded.
